For the last one, I'm going to check my wiring harness. Otherwise, I'm not sure how to solve the other two.
AFAIK, the 1st one results in my NAV system stating "CD/DVD Unreadable" every once and a while ... simply hitting the NAV button, the system continues to function as normal.
