Adding Satelite Radio to an Audi without it from the factory

Start here http://forums.audiworld.com/a6/threads/119038.phtml#496406 but skip the Symphony retrofit. The install for the RNS-E is very similar to the Symphony II retrofit and NSX' post has all the part numbers you need. You will need the 2nd generation SAT receiver of course (check the part numbers quick link for those) and not the first generation as mentioned in the install guide.

Would the install be the same for a 2004 S4 with RNS E? Or is there the chance that my car is pre-wired for Sat already so I just have to connect it?

Do I need the never Sirius receiver 4E0057593 from the new A6 to work fully with the RNS E even though is works wih MMI?

I would guess that the MMI receiver uses MOST and will be no good in an A4.

The North American S4 MY 2004 is pre-wired for SAT (either Sirius or XM). You can simply install the unit in the trunk area, all the wires are there. For Sirius you are looking for part number 8E0 035 593 D or 8E0 057 593 D, other Sirius receivers will likely not work with the RNS-E. If you have the Symphony II or Concert II HU you can use the 8E0 035 593 or 8E0 057 593. The 4E Sirius receiver is only for the A6/A8 MMI systems.
